When filling out your emergency forms, please let us know prior to the start of camp if your child has any of the following:

• Asthma Action Plan

• Diabetes Medical Plan

• Dietary Restrictions

• Medication Authorization Form

• Migraine Headache Plan

• Seizure Emergency Plan

All needed medication is due on the first day of attendance.
